Angina Pectoris
Angina is a symptom of a heart condition known as myocardial ischemia. Simply put, angina occurs when the coronary arteries cannot supply the heart with enough oxygen-rich blood to meet its needs. The symptoms of angina are usually chest pain or pressure and most often appear when the heart requires more oxygen than usual, such as during exercise or a period of stress. Angina symptoms disappear after the patient rests of takes nitroglycerin. This type of angina is known as stable angina. Unlike a heart attack, angina does not cause permanent damage to the heart muscle, but it is a warning symptom of a temporary lack of oxygen to the heart tissue that should be carefully investigated by a physicain to determine its cause and treatment. There are different kinds of angina that do not follow the classic pattern described above. Angina that occurs while the patient is at rest is known as variant of Prinzmetal's angina. Variant angina is caused by spasm in one or more of the coronary arteries, usually a result of atherosclerosis(hardening of the arteries). Another common type of angina is unstable angina, in which stable angina occurs more frequently and may not be relieved by rest or medication. Unstable angina can occur at rest or during exercise or stress. Patients with angina should be aware of their symptoms and contact their physician if these symptoms change or worsen.
Being aware of changes in or worsening of angina symptoms is important, since stable angina can progress to unstable angina, heart attack, or sudden death due to abonormal heart rhythm. For many patients, good control of angina symptoms depends on their taking their medications correctly.

심장으로 산소공급이 차단된다.
협심증은 심근경색증으로 알려진 심장질환의 한 증상이다. 단순하게 말하면 협심증은 관상동맥이 심장의 수요를 만족시킬 수 있을 만큼 충분하게 산소가 풍부한 혈액을 공급할 수 없을 때 발생한다.
협심증의 증상은 보통 가슴 통증(흉통) 또는 압박감이며, 운동시 또는 스트레스 기간 동안과 같이 심장이 평상시에 비해서 더 많은 산소를 필요로 할 때 가장 자주 나타나게 된다. 협심증 증상은 환자가 안정을 취하거나 nitroglycerin을 복용하면 소실된다. 이러한 형태의 협심증을 안정형 협심증(stable angina)이라고 지칭한다.
심장마비와 달리 협심증은 심장근육에 영구적인 손상을 유발하지는 않지만 심장근육의 일시적인 산소 부족을 경고하는 증상이며, 이는 의사에 의해 면밀한 검사를 받아서 그 원인을 찾고 치료를 받아야 한다. 앞에서 언급한 전형적인 양상을 따르지 않는 다양한 종류의 협심증이 있다.
휴식시에 발생하는 협심증은 이형 협심증(variant angina) 또는 Prinzmetal형 협심증으로 칭한다. 이형 협심증은 하나 이상의 관상동맥의 경련에 의해서 유발되며, 보통 죽상경화증(동맥이 굳는 것)에 의해 유발된다.
다른 흔한 형태의 협심증은 불안정형 협심증(unstable angina)으로 이는 안정형 협심증이 더 빈번히 발생하는 경우와 휴식이나 약물에 의해서 경감되지 않는 상태를 지칭한다. 불안정형 협심증은 휴식시, 운동 중 또는 스트레스 하에서 발생할 수 있다.
협심증 환자는 그들의 증상을 자각하고 있어야 하며, 이들 증상들이 변하거나 악화될 경우에는 의사를 만나야 한다.
증상을 자각하고 있어야 한다.
협심증은 질환이 아니며, 심장근육(심근)이 일을 수행할때 산소가 충분한 혈액을 얻지 못하는 질환(경색증)의 한 증상이다. 이 질환의 원인은 여럿이 있을 수 있으나 가장 흔한 원인은 죽상경화로 인해서 관상동맥이 차단되는 것이다. 대부분의 환자에서는 협심증이 평상시에 비해 심장이 산소를 더 많이 요구할 때에만 발생한다. 이러한 상황은 운동시, 스트레스 또는 감정이 격할 때, 온도나 고도가 극단에 달할 때, 과식 후, 흡연 중에 발생할 수 있다. 대부분의 협심증 발작은 아침 6시와 정오 사이에 발생한다.
전형적인 증상:협심증은 미국인 6백만 이상이 앓고 있는 흔한 심장질환이다. 매년 50만명 이상의 환자가 새로 진단된다. 협심증의 위험성이 높은 환자는 당뇨의 가족력이 있는 경우, 죽상경화증의 위험성이 높은 경우, 고혈압, 고콜레스테롤혈증, 당뇨, 흡연자이다.
협심증의 증상이 있는 환자들은 종종 가슴 뼈(흉골) 바로 밑 부분의 통증 또는 압박감을 호소한다. 이러한 느낌은 왼쪽 어깨, 팔, 턱 또는 치아가 있는 윗부분과 등뒤로 전이될 수 있으며, 덜 흔하게는 오른쪽 팔 아래로도 전이될 수 있다. 이는 대부분 운동시, 특히 식사 후 운동시에 발생하기 시작한다.
이는 또한 따뜻한 방 안에서 춥거나 바람이 부는 밖으로 걸어나올 경우에 발생할 수 있다. 또한 협심증 통증은 두려움, 분노 또는 수면 중 꿈을 꾸는 동안과 같이 강렬한 감정에 의해서도 유발될 수 있다. 협심증의 제 증상은 환자가 심장마비에 대한 두려움을 갖도록 할 수 있으나 전형적인 안정형 협심증인 경우 휴식을 취하거나 nitroglycerin을 설하로 투여함으로써 흉통이 사라지게 된다.
진단:환자의 병력, 이학적 검진과 심장근육에 나타난 심근허혈의 정도를 확인할 수 있는 다양한 검사를 통해서 진단이 내려진다. 다수의 환자에서 검사는 운동 스트레스 부하 심전도 또는 스트레스 부하 심초음파 검사로 시작된다.
관상동맥 혈관조영술과 같은 더 침습적인 검사가 관상동맥 차단 정도에 대한 보다 상세한 정보를 제공할 수 있다. 정교한 단층촬영 이미지(PET 및 SPECT 스캔)를 사용하면 심장을 통하는 혈류를 볼 수 있다.
협심증이 있는 흡연가들은 흡연을 중단해야 하며, 심장에 추가적인 스트레스를 가하지 않기 위해서 고혈압은 치료되어야 한다. 콜레스테롤 농도를 낮추는 것이 더 이상의 관상동맥 죽상경화증을 예방할 수 있다. 잘 디자인된 운동 프로그램 또한 바람직하다.
약물치료:협심증을 치료하는 데 사용되는 약물 종류는 nitrates, 베타 차단제, 칼슘 채널 차단제가 있다. Nitrate 제제는 관상동맥을 확장시키고, 평활근을 이완시켜서 심장의 산소수요와 공급의 균형을 돕는다. Nitrate는 단시간 지속형 제제와 장시간 지속형 제제가 나와있다. 단시간 지속형 nitroglycerin은 혀 밑에서 녹아서(설하) 협심증 통증 또는 압박감을 신속히 경감시켜 준다.
대부분의 환자들은 1~3분 이내에 통증 경감을 느끼기 시작한다. 설하정은 추가 투여시 5분 간격으로 총 3회까지 투여할 수 있다. 단시간 지속형 경구 스프레이 제제도 있다. 이들 단시간 지속형 nitrate 제제는 협심증 발작시에 사용할 수 있도록 항상 소지하고 다녀야 하며, 약효를 보증하기 위해서는 자주 새로운 약으로 처방받아야 한다. 협심증 발작을 예방하기 위해서 사용하는 장시간 지속형 nitrate 제제로는 경구용 isosorbide, nitroglycerin 연고, 피부패취 제제가 있다.
