Using a Peak Flow Meter
Device Measures Air Flow
For persons who have asthma, a small, portable device known as a peak flow meter can be a very important tool in monitoring the disease. This device measures how fast an individual can blow air out of his or her lungs after taking a deep breath. The peak flow meter is actually measuring obstruction in the airways. The meter can be an important early warning device indicating that asthma is getting worse. It can warn of this change even a day or two before there are any symptoms. Knowing that airway obstruction is getting worse can help asthma patients adjust dosing of medicines to avoid emergency room visits or hospitalizations due to asthma. The peak flow meter is easy to use. The value measured, called the peak flow rate, can be compared to each patient's "personal best" value to determine how well asthma is controlled. A pharmacist or doctor can show asthma patients how a peak flow meter can help in managing asthma.

천식 환자들에게 작고 휴대하기 용이한 최대 호기류 속도 측정기는 질환을 모니터링에 할때 아주 중요하다. 이 기구는 사용자가 최대로 숨을 들여 마신 후 얼마나 빨리 숨을 내쉴 수 있는가를 측정하는 도구이다. 실제로 최대 호기류 속도 측정기는 기도의 폐색을 측정한다. 이 기구는 천식이 심해짐을 알아낼 수 있는 중요한 조기 경고기구이다. 최대 호기류 속도(PEF) 측정기는 어떤 증상이 나타나기 하루 혹은 이틀 전에 미리 그 변화를 알아낼 수 있다. 천식환자들은 기도의 폐색이 악화되고 있다는 것을 스스로 알아냄으로써 약물용량의 조절 등을 통해 응급실이나 병원신세를 지는 것을 면할 수 있다. PEF 측정기는 사용이 용이하다. PEFR이라고 불리우는 측정치는 그 사람의 최대 호기류 속도와 비교하여 천식이 얼마나 잘 조절되고 있는 지를 알 수 있다. 약사나 의사는 PEF 측정기가 환자 스스로 천식을 조절해 나가는 데 얼마나 도움이 되는지에 대해 잘 알려줄 수 있다.
최대 호기류 속도 측정기 사용법
1. PEF 측정기의 오르내림바를 계측기 제일 아래쪽에 둔다.
2. 선 자세로 숨을 들이킬 수 있는 한 최대한 들이 마신다.
3. 측정기를 입에다 대고 입술로 마우스피스(입으로 무는 기구)를 꽉 맞게 문다. 혀는 마우스피스에 닿지 않게 한다.
4. 곧바로 서서 숨을 가능한 한 빠르고 강하게 불어낸다.
5. 오르내림바가 위치한 수치를 기록한 후 두번 더 실시한다.
6. PEFR 기록표에 세번 중 수치의 최고값을 기입한다.
천식의 정도 측정
PEF 측정기는 작고 휴대하기 용이한 도구로 숨을 최대로 들이마신 다음 가능한 한 빨리 힘껏 불어서 폐에서 불어낼 수 있는 최대의 공기속도를 측정하는 기구이다. 이것은 병원에 있는 폐활량 측정기와 비슷하지만 가정용으로 고안된 기구이다. 측정범위가 낮은 기구는 일반적으로 어린이에게 권고되며, 표준범위의 측정기는 청소년과 어른들에게 적합하다. 어떤 측정기는 모든 연령의 어린이들이 사용할 수 있도록 고안되었다.
최대 호기류 측정기의 용도:의사들이 PEF 측정기를 권유하는 가장 흔한 이유는 천식의 심한 정도를 단지 그 증상에 의존하는 것이 아니라 보다 정확하게 결정하기 위해서이다. PEF측정기를 사용하면 폐기능의 악화를 미리 알게 되고 의약품의 용량을 적절히 조절할 수 있어 응급실이나 병원에 입원하는 상황을 피할 수 있게 된다. 보다 빠른 치료는 보다 적은 양의 의약품으로 보다 빨리 정상호흡으로 복귀함을 의미한다. PEF 측정기의 다른 용도로는 야간 천식, 운동 유발성 천식의 진단, 다음 번 의사 방문 사이에 만성 천식의 치료 모니터, 천식발작 동안의 의약품 효과 검증, 천식을 악화시키는 `인자' 확인 등과 천식의 계절별 변화 확인 등이 있다.
사용자의 최대 폐활량 상태:최대 호기류 속도(PEFR)는 신장, 성별, 나이 등으로 예측가능 하지만, 의사들은 천식 조절을 위한 지침으로 각 `개인의 최대 호기류 속도'를 정하기를 원한다. 이 수치를 측정하기 위해서는 2주일 동안 한낮이나 이른 저녁시간에 측정한 최고치를 기록해야 한다(기관지 확장제는 사용 후에 측정한다). 아래의 지침을 사용함으로써 그 측정기 위에 선이나 테이프를 사용해서 그 구간을 표시한다. 이러한 구간들은 환자와 의사가 천식이 잘 조절되는지 그리고 악화된다면 차후 어떤 행동을 선택할 것인가를 결정하는 것을 돕는다.
녹색구간 `all clear':각 개인의 최대 호기류 속도 80~100%:치료 계속
노란구간 `caution':각 개인의 최대 호기류 속도 50~80%:처방된 약물 조정
빨간구간 `medical alert':각 개인의 최대 호기류 속도 50% 이하:즉시 기관지 확장제를 흡입하는 것이 요구된다. 그리고 그 수치가 노란구간 또는 녹색구간으로 즉시 돌아가지 않으면 의사를 방문한다.
측정치의 기록
PEF 측정기는 의사가 권하는 만큼 자주 사용해야 한다. 하루에 몇 번(3~5)이거나, 한 주에 2~3번일 수 있다. 또한 천식이 악화된다고 느낄 때마다 PEFR을 측정할 수 있다. 많은 의사들은 일반적으로 PEF 측정기에 나타나는 최대의 PEFR(폐에서 불어낼 수 있는 최대의 공기속도) 숫자를 PEF기록지나 그래프에 기록할 것을 제안한다. 이러한 정보는 의사에게 진찰받는 사이에(먼저 진찰과 다음 진찰의 간격) 천식을 모니터링하는 환자나 의사 모두에게 도움을 줄 것이다. 만일 PEFR이 노란 또는 적색 구간에 있다면 약물 조정에서 어떤 방법을 택할지 PEF 기록지에 명확히 표시해야 한다. 환자의 최대 폐활량의 수치는 변할 수 있고 의사가 직접 해마다 한두 번을 측정해야만 하고 의사는 그 구간에 상응하는 조절을 할 수 있다. 부정확한 측정을 피하기 위해서 PEF 측정기는 순한 세제와 뜨거운 물로 측정기 제조회사가 지시하는 대로 정기으로 세척해야 한다.
